Evaluation Information
Following is a guide to scheduling your child’s evaluation. Questions? Please reach out to your family resources coordinator (FRC).
Once your referral is placed either by you or by your provider, our Enrollment Coordinator will reach out via phone to collect and verify information about you and your child.
The Enrollment Coordinator will also schedule your intake meeting with your Family Resources Coordinator, as well as your child’s Evaluation with our team of specialists.
The intake meeting typically takes place virtually, but if you need an in person meeting, please ask.
Our evaluations take place in your child’s natural environment, most typically in your home. If you need other options, please let us know.
If you have opted in for text messaging, you will also receive text messages to remind you of the scheduled appointments, and also to confirm them.

Our evaluations are scheduled in the mornings and early afternoons on weekdays
We do bill your insurance for the evaluation. In the event that your insurance does not pay, you will not receive a bill from us. There is no out-of-pocket expense to you for this evaluation.
The evaluation takes between 60 and 90 minutes
If there is a reason that you are not able to keep your child’s appointment, we do ask for at least 24 hours notice whenever possible. Please call 253.471.2727 and press Option 2 to reach our scheduling department. If they are on another call, please leave a voicemail with your child’s information and evaluation date/time.
Please note: that current availability can sometimes change on a daily basis. The need to reschedule your child, could result in a delay of the timeline in which your child would receive services. We will make every effort to reschedule your child’s evaluation in a timely manner.
